This is one reason knee evaluation should not focus only on the meniscus, cartilage, or ligaments. Examination may include assessment of hip strength and mobility, lumbar and pelvic mechanics, gait, foot and ankle motion, tendon loading, patellar tracking, and the specific activities that reproduce symptoms.

When symptoms suggest a regional or movement-related contributor, treatment may emphasize rehabilitation strategy, corrective exercise, osteopathic evaluation and treatment, activity modification, bracing when appropriate, medication strategy, or selective image-guided treatment when clinically appropriate.

Sports-Related Knee Pain

Knee pain is common in active adults, runners, lifters, recreational athletes, and people who participate in court sports, skiing, cycling, dance, yoga, and combat sports, including Brazilian jiu-jitsu, wrestling, boxing, kickboxing, and mixed martial arts.

Sports-related pain may develop suddenly after a twist, fall, impact, or awkward landing. It may also develop gradually from repetitive loading, training volume, strength imbalance, mobility restrictions, or insufficient recovery.

Evaluation focuses on the location of pain, mechanism of injury, swelling, instability, locking, catching, range of motion, strength, sport-specific demands, and whether symptoms suggest a joint, tendon, ligament, bursal, or referred source.

How Knee Pain Is Evaluated

A focused knee evaluation typically includes a discussion of the pain pattern, onset, aggravating activities, prior injuries, training history, previous treatment, and functional limitations. The examination may assess range of motion, swelling, tenderness, ligament stability, meniscal signs, patellar tracking, tendon loading, hip and ankle mechanics, gait, and related spine or pelvic contributors when relevant.

Diagnostic ultrasound may be used when appropriate to evaluate selected superficial structures around the knee, such as tendons, bursae, effusions, cystic changes, or soft-tissue irritation. X-rays or MRI may be considered when the history, examination, or symptom course suggests that additional imaging would change management.

Imaging is useful when it answers a clinical question. It is not a substitute for a careful examination.

Treatment Planning Based on the Diagnosis

Many knee pain conditions can be managed without surgery, but the right plan depends on the specific diagnosis, severity of symptoms, activity goals, prior treatment response, and whether the knee shows signs of instability, swelling, mechanical symptoms, or progressive limitation.

Treatment may include activity modification, targeted rehabilitation, bracing, medication when appropriate, osteopathic assessment, diagnostic ultrasound, image-guided injections, or referral for orthopedic evaluation when needed. Selected persistent patellar or quadriceps tendon conditions may also be considered for percutaneous tendon and soft-tissue procedures after diagnosis-driven evaluation. In certain knee pain patterns, genicular nerve blocks may be considered when sensory nerves around the knee appear to contribute to pain.

For active patients, the goal is not simply to 鈥渞est鈥 the knee. The goal is to identify what can be modified, what should temporarily be avoided, and what can safely continue while the underlying problem is addressed.

Osteopathic, Physiatric, and Biomechanical Approach

Knee pain is often influenced by mechanics above and below the knee. Hip mobility, pelvic control, lumbar spine mechanics, ankle mobility, foot mechanics, strength deficits, and training patterns can all affect knee loading.

Advanced Injection Options

For selected patients, orthobiologic treatment options may be discussed as part of a broader knee-care plan. These treatments are considered only after a clinical evaluation and depend on the diagnosis, severity of arthritis or tissue injury, imaging findings, prior treatment response, activity goals, and overall medical context.

Orthobiologic options are not appropriate for every patient and are not presented as cures, cartilage-regrowth treatments, or guaranteed alternatives to surgery. They are generally considered selectively, not as first-line treatment for all knee pain.

Viscosupplementation for Knee Arthritis
