Insurance & Payment
Many patients use their commercial PPO or POS insurance benefits for care at 91ÊÓÆµ. Depending on the individual plan and deductible status, benefits may cover a substantial portion—and in some cases most or all—of the eligible cost of care. 91ÊÓÆµ is non-participating with most insurers. Coverage, reimbursement, and patient responsibility vary by plan and cannot be guaranteed.
Complimentary Benefit Review
Before scheduling, our office can review available out-of-network benefit information for an applicable commercial PPO or POS plan and explain the anticipated financial arrangements. There is no charge for this review.
Benefit information is based on information reported by the insurer and is not a guarantee of coverage or payment. For applicable plans, our office generally submits claims on the patient’s behalf and assists with related billing logistics.
Same-Day and Next-Day Appointments
Same-day and next-day appointments may be available. When benefit verification cannot be completed before an immediate appointment, the patient may elect to proceed by paying the stated amount in advance. Any expected payment will be explained before the appointment is confirmed.
If applicable out-of-network benefits are subsequently confirmed, our office may submit the claim. If the insurer pays toward the visit, that payment will be applied to the patient’s account and the account will be reconciled based on the insurer’s payment and the patient’s remaining responsibility.
Depending on the amount paid by the plan, some or all of the amount collected in advance may be refunded. Any resulting credit balance will be returned to the patient.
Self-Pay and Payment Options
Self-pay options are available for patients without applicable insurance benefits and may also be considered by patients with high-deductible plans. Expected charges are reviewed before care.
Cigna patients are seen on a self-pay basis. The office can provide an itemized invoice for patients who wish to seek reimbursement directly from Cigna but does not submit or manage Cigna claims. Reimbursement depends on the individual plan and is not guaranteed.
HSA, FSA, and HRA cards may be used for eligible services.
Plans Not Accepted
91ÊÓÆµ does not accept or bill Medicare, Medicare Advantage, Medicaid, Workers’ Compensation, or No-Fault.
